In this article, drawing on my experience of more than 600 cases, I explain exactly what BBL means, what the operation involves, who is a candidate, how much it costs in Barcelona and —above all— how it is performed safely following the protocol updates of 2018.
What does BBL stand for?
BBL stands for Brazilian Butt Lift. The name comes from the Brazilian school of plastic surgery, which pioneered and perfected the technique of fat transfer to the gluteal area during the 1990s.
In technical terms, BBL is synonymous with:
- Gluteal fat transfer
- Gluteal lipofilling
- Gluteal fat grafting
- Autologous buttock augmentation (using the patient's own fat)
All these terms describe the same procedure. The key difference from buttock augmentation with implants is that the BBL uses no prostheses — only the patient's own fat.
What exactly is the BBL?
The BBL is an aesthetic surgery that combines two procedures in a single operation:
- Liposculpture of the areas with excess fat (abdomen, flanks, upper back, lower back, thighs).
- Gluteal fat grafting using the harvested and purified fat.
This dual action is what makes the BBL so sought-after: it not only augments and projects the buttocks, but also sculpts the waist and redefines the overall body contour simultaneously. It is the surgery that most effectively produces the "hourglass" figure.
How is the BBL operation performed?
The procedure takes between 3 and 5 hours under general anaesthesia or deep sedation. It is performed in an accredited hospital operating theatre, not in an outpatient clinic. The phases are as follows:
Phase 1: marking and planning
With the patient standing, the donor areas (from which fat will be harvested) and the recipient areas (the gluteal quadrants where it will be injected) are marked. This ensures the natural symmetry of the body is respected.
Phase 2: liposculpture
Using 3–4 mm cannulas with Vaser (ultrasound) or PAL (vibration) technology, fat is harvested from the donor areas. Between 2 and 5 litres of adipose tissue are obtained depending on the case.
Phase 3: fat processing
The fat is filtered and purified to separate viable adipose cells from serum, blood and debris. Only between 50% and 70% of the extracted volume is actually suitable for grafting.
Phase 4: gluteal fat grafting
The purified fat is injected into the subcutaneous plane (never deep intramuscular) using blunt cannulas and, in many centres, ultrasound guidance. It is distributed across multiple tissue planes to optimise cell survival.

Who is a candidate for a BBL?
Not everyone is a suitable candidate. For the BBL to be safe and effective, three conditions must be met:

Very slim individuals (BMI < 22) generally do not have enough fat to harvest and are poor candidates: the result is modest and does not justify the surgery. In these cases we consider gluteal implants or a combined approach.

Is the BBL dangerous? The truth about safety
Until 2017, the BBL was the aesthetic surgery with the highest mortality rate worldwide: 1 in every 3,000 cases according to the Multi-Society Gluteal Fat Grafting Task Force study. The cause was almost always the same: pulmonary fat embolism resulting from fat injection into deep muscular planes near the gluteal veins.
Since 2018, international bodies (ISAPS, ASPS, ASAPS, FILACP) updated their protocols with three non-negotiable rules:
- Exclusively subcutaneous injection — never into the gluteal muscle.
- Blunt, rigid cannulas — not flexible ones.
- Visual or ultrasound monitoring of cannula depth throughout the procedure.
Applying these protocols has reduced the rate of serious complications to levels equivalent to those of an abdominoplasty. To confirm that your surgeon follows them, ask directly: "In which plane do you inject the fat? Do you use ultrasound guidance?" If the answer is not clear, seek a second opinion.
BBL recovery
BBL recovery is demanding during the first 4–6 weeks because the survival of the transferred fat depends on avoiding pressure. Here is what to expect:

How much fat is retained long-term?
Of all the fat injected, 70% remains permanently after 6 months, once it has fully vascularised. The remaining 30% is reabsorbed during the first 3–6 months. This is why we slightly overcorrect during surgery to compensate for the anticipated loss.
The most decisive factor for fat "taking" is maintaining a stable weight: if the patient loses 5+ kilograms during the first year, they will proportionally lose gluteal volume. If they gain weight, they will gain volume but also in other areas. The recommendation is not to vary weight by more than 2–3 kilograms during the first year.
