Pilar’s case
Pilar, nearly 70, had excess weight and a very drooping double chin. The accumulation of fat in the submental and cervical area gave her neck and facial oval a more aged and heavy appearance.
She did not want to undergo a major surgery such as a facial or cervical facelift, but rather find a solution with a more comfortable recovery and no large scars. She wanted to restore the definition of her facial oval in a natural, discreet way.
Options considered
Initially, the possibility of a facelift – at least at the cervical level – was proposed to tighten the skin and reposition the neck tissues. This technique would have offered very comprehensive results in terms of skin tightening.
However, Pilar declined this option as she did not want a longer procedure, hospitalisation, or the scarring that a facelift typically entails. An alternative was therefore sought that respected her priorities.
Treatment performed
As an alternative, chin liposuction with skin retractors was proposed. It is performed through three very small incisions, hidden behind the ears and below the chin, leaving virtually imperceptible scars.
Very fine cannulas (approximately 2.5 mm) are introduced through these incisions to aspirate the entire submental and cervical region, removing the maximum amount of accumulated fat.
Once the liposuction is complete, a very fine and short skin retractor – specifically adapted for the neck area – is applied. It is a less intense retractor than the body version, since in this region it is essential to respect the key structures: blood vessels, nerves and especially the carotid arteries. The retractor helps the skin to contract and adapt better to the new contour.
Outpatient and minimally invasive technique
- Use of fine cannulas (~2.5 mm) for precise work in the neck area.
- Gentler retractor, designed to work safely where the vascular and nerve structures are especially delicate.
- Minimally invasive and outpatient surgery, of short duration.
- The patient can return home the same day.
- Simpler post-operative period than a classic facelift.
The three small incisions are hidden behind the ears and under the chin, and are virtually imperceptible once healed.
Result
After chin liposuction with retractors, Pilar has a much more defined neck and a lighter facial oval, with the double chin noticeably reduced. Dr. Martín Gastón describes the result as “more than pleasing”: a very visible improvement of the contour without the need for a facelift and, above all, without scarring.
For a patient who did not want a major procedure, this option has allowed her to rejuvenate the neck area harmoniously while maintaining a natural appearance.
If the patient were to follow a nutritional plan and lose a little more weight, the loss of facial fat would make the result even more remarkable.
